Eligibility for Burial in a VA National Cemetery

 As a Veteran you are likely eligible for burial in VA National Cemetery. The following is a "APPLICATION FOR PRE-NEED DETERMINATION OF ELIGIBILITY FOR BURIAL IN A VA NATIONAL CEMETERY". IMPORTANT: Pre-Need means before death. Only complete this form if you are applying for a Pre-Need determination of eligibility for burial. Click this link VA Form 40-10007 to download and print document VA Form 40-10007. Then Submit your Application and Supporting documents to VA by Mail: to NCA Intake Center, P.O. Box 5237, Janesville, WI 53547; or by Fax: to the National Cemetery Scheduling Office at (855) 840-8299.
